# How to create a HR form?

You can use IceHrm Form for different purposes. Such as;

> 1\. To request employee surveys

> 2\. Employees’ opinion on something

> 3\. To get probation review forms filled

> 4\. To process employee existing forms filled

Let’s see how to request employee surveys using [IceHrm](https://icehrm.com).

1\. Log into the **Admin Account**

2\. Go to **Manage -> Data Collection**.

3\. OR you can click on to the **HR Form** module on the **Dashboard**

4\. Under **HR Forms** tab, click onto **Add New** option

5\. Give a **meaningful name** and **Description** based on your requirements

6\. Click on the **Add Field** option

7\. As the **Field Label**, you can add **a question** or a **topic** which you would like to request from the employee

8\. Then **Select** a field label according to your requirement

> You can select the Text Area, Text Field, Select option or even you can request your employees to upload a file

9\. If you have **Selected** the **Select** option, you can give the **Options** under the **Options** tab

10\. Then click on **Done**

> You can repeat the same steps if you have a number of questions to ask from your employees.

12\. Once you are done with adding fields, you can click on **Save**
